Description: Officer Chadwell, accompanied by three other law enforcement officers, went to serve an arrest warrant. The officers surrounded the residence, identified themselves and entered. The suspect opened fire and the officers responded. The suspect was struck and killed. A bullet from Patrolman David Hunt's weapon ricocheted and fatally wounded Officer Chadwell. (friendly fire)
