Description: Officer Cook was killed in an automobile accident while he and his partner, Officer Nick Ferrando, responded to a domestic disturbance call. Officer Michael Celis and his partner, Officer David Lee, of the same department were responding to the same emergency call when both patrol cars collided at the intersection of Dolores and 17th Streets in San Francisco's Mission District. Officer Cook received severe head injuries and was pronounced dead at the scene. His partner, Officer Ferrando, was critically injured. Officer Celis and Officer Lee received non-life threatening injuries. Friendly action.
