Description: Officer Costello was a passenger in the (combination ambulance-patrol car), that was carrying a heart attack patient to a local hospital. The ambulance-patrol car was struck by a truck loaded with produce. All five occupants of the ambulance had to be transported to the hospital with injuries. Officer Costello was injured fatally and died of shock and multiple fractures. Officer Frank Krasula was also injured in this accident.
