Description: On November 14, 2014, Police Officer Diaz was injured in an automobile crash when his patrol vehicle was struck by another car. Officer Diaz suffered a head injury and was prescribed various medications. While still on medical leave on July 19, 2016, Officer Diaz fell and lost his balance and injured his elbow which was treated with additional medications. An MRI revealed that the injury required surgery which was performed on September 7th. Officer Diaz was discharged and additional narcotic medications were prescribed. Officer Diaz was found unresponsive at his home and was declared dead at the hospital.
