Description: Sergeant Ortiz Alvarez was accidentally shot and killed while attempting to serve an arrest warrant. Sergeant Ortiz Alvarez and fellow officers were attempting to arrest a suspect wanted for murder. While covering fellow officers, Sergeant Ortiz Alvarez jumped over a fence, placing his M-16 rifle on the ground. When Sergeant Ortiz Alvarez retrieved his weapon, it accidentally discharged. Sergeant Ortiz Alvarez died shortly after arrival at the local hospital.
