Description: Trooper Van Wagenen was investigating a prison break and stopped at a factory to warn employees of the escape. While at the factory, Trooper Van Wagenen tripped and fell on an electric saw blade. Trooper Van Wagenen was transported to the hospital where he died due to blood loss.
