Description: While on routine foot patrol, Watchman Van Duzen was killed when he fell through a grating. The grating had been damaged from a previous accident and was in the process of being repaired. Proper barriers were not installed by construction crews to prevent pedestrians from crossing the weak grating. Watchman Van Duzen?s body was located at the bottom of the vented shaft the following morning.
